Reimbursement Options
All of us at Monarch Medical are sensitive to the ever-spiraling cost of your healthcare.  We congratulate you for recognizing, as we do, that weight management is a central and important medical issue.

The good news is that Monarch Medical does not charge traditionally high office visit rates.  We work hard to keep Monarch’s fees among the most reasonable of your weight management options.  The even better news is that it is becoming much more common for Monarch’s patients to receive some of their clinic charges reimbursed by their insurance carrier.  Medical expertise and affordability – A powerful combination for your long-term success.

While we do not bill insurance companies directly, Monarch’s staff provides you with all the appropriate medical and diagnostic documentation and invoice information that you will be required to submit to your insurance carrier for reimbursement.  Your carefully documented medical information will be prepared after each scheduled practitioner appointment so you can submit it to your insurance carrier.  Our staff can help you with insurance company mailing addresses and contact information.

Please feel free to call the Monarch Patient Care Center (1-866-731-5673) and they can help you understand your options relating to insurance and other reimbursement options. To send insurance reimbursement information, include a copy of your patient invoice ("Superbill") and a copy of your insurance card (both front and back).

Medical Spending Accounts
Employees with medical spending accounts (MSA), set up through their employers, are able to request reimbursement for medical services from their tax-advantaged medical spending accounts. MSAs are an easy and effective way to meaningfully reduce your healthcare costs — allowing you to receive Monarch’s low cost services using pre-tax dollars instead of after-tax dollars (resulting in up to a 45% discount). Benny and other health spending account cards are accepted at all Monarch clinics.

Please verify with your insurance company that the mailing address and claim form provided is correct. Monarch cannot be held liable for address and form changes made by your insurance company.
